I hear some version of that question almost every week. It usually comes from a patient who's just been diagnosed with an autoimmune condition, or who's pretty sure something autoimmune is going on but keeps getting told their labs look "normal." Neither situation feels good. One leaves you with a diagnosis and no explanation. The other leaves you without even that.

Autoimmune disease doesn't just show up one morning. It builds quietly for years before it's loud enough to demand attention, which is exactly why so many patients feel blindsided when they finally get an answer. So I want to walk you through what's actually happening in the body, what causes autoimmune disease to develop in the first place, and why perimenopause tends to be such a turning point for so many women.

What Is Autoimmune Disease, Exactly?

Autoimmune disease happens when your immune system loses the ability to tell the difference between your own tissue and a genuine threat, and starts attacking your body instead of protecting it. Depending on which tissue gets targeted, that shows up as a different diagnosis. Hashimoto's is the immune system attacking the thyroid. Rheumatoid arthritis is the immune system attacking joint tissue. Celiac disease involves an immune reaction to gluten that also damages the small intestine.

The name changes based on the target. The underlying mechanism driving most autoimmune conditions is remarkably similar, and it rarely comes from a single cause.

The Three-Legged Stool of Autoimmune Disease

I like to explain this to patients using an image I come back to often: a three-legged stool. It's a concept from Dr. Alessio Fasano, a gastroenterologist whose research on autoimmunity has shaped how a lot of us in functional medicine think about this. A stool needs all three legs to hold weight. Pull one out and it wobbles. Pull two out and you're on the floor. Autoimmune disease tends to work the same way. It generally takes three things lining up before the disease actually takes hold.

Fasano's research points to three components:

  1. Genetic susceptibility
  2. An environmental trigger
  3. Intestinal permeability, better known as leaky gut

Here's the part I really want you to sit with, though. You don't need to fix all three legs to feel things shift. Strengthen even one, and the whole stool gets more stable. I've had patients see antibody levels drop and symptoms ease up meaningfully just from working on one leg, long before we've touched the others. Progress doesn't require perfection here.

Let's break each one down.

Leg One: Genetic Susceptibility

Certain genetic variations raise your odds of developing specific autoimmune conditions. The HLA-DQ genes, for instance, are strongly tied to celiac disease risk. But carrying the gene isn't a life sentence. It just means your threshold is lower, so it takes less from the other two legs to tip you over.

This is where something called nutrigenomics comes in handy. It's the study of how your particular genetic makeup interacts with what you eat, and it can point us toward which foods and habits are working with your biology instead of against it. You can't rewrite your genes. You can absolutely change how they get expressed, which honestly matters more.

Leg Two: Environmental Triggers

Ask a patient what was going on in their life or health right before symptoms started, and more often than not, they can name something. A brutal virus that never quite resolved. A mold-infested rental. A divorce that upended everything. Sometimes it's obvious. Sometimes it takes a little digging.

Common triggers I see include:

  • Infections like Epstein-Barr virus, herpes simplex, or H. pylori
  • Mold and mycotoxin exposure
  • Heavy metals, whether from old dental amalgams or environmental sources
  • Chemical exposure, including herbicides and pesticides
  • Emotional trauma, like the loss of a relationship, a job, or someone you love
  • Physical trauma or a severe illness that knocked you flat

Sometimes there's one clear culprit. More often it's a pile of smaller stressors that eventually overwhelmed your body's ability to keep up. Either way, naming the trigger and addressing it head on is part of what shortens this leg back down.

Leg Three: Leaky Gut (Intestinal Permeability)

This is the leg I spend the most time on with patients, mostly because it's the one we can actually do something about right now.

Your gut lining is made of cells held together by tight junctions, structures that work a bit like grout between tiles. Healthy grout keeps everything sealed and only lets fully digested nutrients through into your bloodstream. Damaged grout lets things through that have no business being there, like undigested food particles, bacterial fragments, and toxins.

Chronic stress, frequent antibiotics, NSAIDs, a diet full of processed food, alcohol, and glyphosate exposure all wear down that lining over time.

And here's where it gets genuinely strange. Some of what leaks through, like gliadin from wheat or casein from dairy, looks structurally similar to certain tissues in your body. When your immune system builds antibodies against these proteins, those same antibodies can end up attacking your own tissue too, simply because the shapes are close enough to confuse. This is called molecular mimicry, and it's a big part of why gluten and dairy sensitivities show up so often alongside thyroid autoimmunity. Gliadin and thyroid tissue apparently look enough alike that the immune system genuinely can't tell them apart.

Of all three legs, this one gives us the most to work with. You can't undo your genetics, and past triggers are already behind you. But the gut lining can heal, and when it does, patients usually feel the difference before their labs even catch up.

Why Perimenopause Raises Autoimmune Risk

Here's something I wish got talked about more. Women develop autoimmune disease at far higher rates than men, and estrogen is a big reason why. It's not just a reproductive hormone. It also helps regulate immune function, including how well your body maintains tolerance toward its own tissue.

During your reproductive years, estrogen rises and falls in a fairly predictable pattern. Perimenopause throws that pattern out the window. Instead of a smooth decline, estrogen starts swinging, spiking unusually high one cycle and crashing the next, sometimes within the same month. That instability, more than the eventual drop itself, seems to be what really destabilizes immune regulation.

Estrogen helps keep regulatory T cells doing their job, and their job is essentially crowd control for your immune system, keeping it from turning on your own tissue. When estrogen is bouncing around unpredictably, that regulation gets shakier. At the same time, estrogen also helps maintain those tight junctions in your gut lining we just talked about. So you can end up with a gut that's more permeable at the exact moment your immune system is least equipped to handle it.

In stool terms, perimenopause has a way of rattling two legs at once. That combination is a big part of why so many women get their first autoimmune diagnosis during this window, or watch a condition like Hashimoto's that had been quiet for years suddenly flare back up. You're not imagining it, and it's not just "getting older." It's a real, measurable shift, and it deserves more attention than "let's just track your cycle and see how you feel."

How to Heal Leaky Gut and Support Autoimmune Recovery

The encouraging part is that leg three, your gut lining, can be repaired even while your hormones are in flux. I use what's often called the 5R approach with patients working through gut-driven autoimmune issues:

Remove the things provoking inflammation, whether that's specific foods like gluten or dairy, or an underlying issue like SIBO or a lingering infection.

Restore proper digestion, which sometimes means supporting stomach acid or enzyme production if your body isn't breaking food down the way it should.

Reinoculate with beneficial bacteria through targeted probiotics, prebiotic foods, and fermented foods that help rebuild a healthy gut ecosystem.

Repair the lining itself with nutrients like L-glutamine and zinc-carnosine, which support rebuilding those tight junctions.

Rebalance the daily habits that keep the other four steps working, since stress, sleep, and movement all influence how well your gut functions over the long haul.

None of this comes as a one-size-fits-all handout in my practice.
